Fear is debilitating.  The first thing you need to do is to STOP judging yourself.  All the 'shoulds' in the world are not going to help or change anything.  

Secondly, if you are safe, you need to start giving yourself permission to feel what you feel.  Give yourself permission to be where you are emotionally.  "It's ok to be afraid."  Give yourself permission too, to want what you want.  "It's ok to want to be happy.  It's ok to want to leave.  It's ok to want to be free.  It's ok to want to be safe.  It's ok to want to be loved ina healthy way." 

You deserve all those things and more, you just don't believe it yet.  I would recommend again professional help, and if I may, a wonderful book by Louise Hay called You Can Heal Your Life.
